Form - Please fill out this demo form

This form is an example of creating a Zamic form, sending it to a customizable Zamic site with it's own domain name. This form does nothing when you submit it, but it can save the data to a database that can later be searched and manually exported or exported based on a schedule. The form has the ability to email the form data to an email address in a basic table or based on a template to a single email address or a selection of email addresses.
